The question we get asked constantly at our Shoreditch clinic: Should I get filler or try microneedling? The honest answer might surprise you — especially if you're used to the UK beauty industry's approach to anti-ageing.
The Western Approach vs the Korean Approach
In the UK, injectables have been the default anti-ageing solution for years. Filler is fast, the results are immediate, and the industry around it is enormous. But if you look at what's happening in Korean dermatology clinics — the clinics that the rest of the world's beauty industry copies — you see a very different picture.
Korean dermatologists overwhelmingly favour treatments that stimulate the skin's own regenerative processes. The philosophy is simple: a face that looks rested and youthful because of what the skin is doing naturally will always look better than a face that looks altered.
What Filler Actually Does
Dermal filler (typically hyaluronic acid gel) is injected beneath the skin to add volume, smooth lines, or reshape facial contours. The results are immediate and can last 6 to 18 months depending on the product and area. It's a legitimate treatment with real benefits for specific concerns — particularly deep volume loss.
But filler doesn't improve the skin. It adds to it. The texture, tone, and quality of the skin itself remains unchanged. And with repeated use over years, filler can migrate, distort proportions, and create an appearance that reads as 'done' rather than refreshed.
What Microneedling Does
Microneedling (also called Collagen Induction Therapy) works by creating thousands of controlled micro-injuries in the skin. These trigger your body's natural healing response — producing new collagen and elastin, the proteins responsible for firm, plump, youthful skin.
The results develop over 4 to 6 weeks as that new collagen forms. What you get is improvement in the skin's actual structure: finer texture, softened lines, reduced scarring, and a natural plumpness that looks completely like you — just rested and restored.
The Key Differences
- Filler adds volume externally. Microneedling builds it from within.
- Filler results are immediate. Microneedling results develop over weeks and improve with each session.
- Filler carries risks of migration, asymmetry, and vascular complications. Microneedling has a 24-48 hour recovery (mild redness) and very low risk profile.
- Filler needs topping up every 6-18 months. Microneedling stimulates ongoing collagen production that outlasts the treatment itself.
- Filler doesn't change skin quality. Microneedling dramatically improves it.
Who Is Microneedling Best For?
Microneedling is particularly effective for:
- Fine lines and early signs of ageing
- Acne scarring and post-breakout marks
- Enlarged pores and uneven texture
- Dull, tired or lacklustre skin
- Anyone who wants results that look natural
- Anyone who has had filler before and wants to reduce their dependence on it
What Do Korean Dermatologists Recommend?
Korean dermatology clinics typically use microneedling as a cornerstone treatment — often in combination with PDRN serums (salmon DNA extracts that turbocharge the skin's healing response), hyaluronic acid infusions, and LED therapy. The approach is layered, progressive, and focused on building the best possible version of your own skin.
Filler is used selectively in Korean clinics — for significant volume loss, typically, and in much smaller quantities than has become normalised in the UK market.
